Context
A hospital is a technical building before it is a public one.
Public hospital investment in France is running at an unusual level, and the programmes it funds are almost all renovations of buildings that stay in operation throughout. That single constraint, working on a site that cannot close, governs the engineering far more than the nominal scope does.
Medical fluids illustrate it. Oxygen, medical air and vacuum networks are life-critical and cannot be interrupted, so every modification is designed around a phased switchover with redundancy, not around the shortest route. The same reasoning applies to ventilation in operating theatres and to electrical redundancy: continuity of service is the design brief.
Regulation
Thermal, acoustic and air quality apply in full.
A hospital renovation carries the same obligations as any other tertiary building. The tertiary decree imposes a measured reduction trajectory on the estate, declared annually through OPERAT. Where the trajectory is unreachable for reasons inherent to the activity, and intensive care or imaging often qualifies, a modulation can be substantiated, but it has to be built on measurement rather than asserted.
Acoustic comfort is a clinical parameter in wards and a confidentiality requirement in consultation rooms. Indoor air quality matters for the same reason. Both are measured on the finished building, which is why we treat them as design commitments rather than as end-of-works checks.
